• High School or College Transcripts

• 3 Letters of Recommendation

• 2 Page Statement of Purpose

Thank you for applying!  Here is some more information about the scholarship that may be useful to you:

• The distribution of scholarship monies ($1,000) is at the sole discretion of the OMIA Foundation Board of Directors

• The check will be written to the university or college at which the student attends

• One scholarship will be distributed per academic year

• The 2009 / 2010 scholarship winner will be chosen in Spring 2010 and the monies will be mailed to the winner during June 2010

• Application deadline for this scholarship is February 1, 2010
